Where do I find my health insurance fund code?

The code of your health insurance fund (9 digits) is at the top of your certificate of entitlement, in the “Organisme de rattachement” box.

Where to find it online

  • Download the certificate of entitlement from your ameli.fr account: the code is printed on it.
  • Top-up insurers (mutuelles) ask for it to set up automatic claims transfer (“Noémie”) between social security and the top-up health insurance.

If you have lost it

  • Generate the certificate of entitlement again from the ameli account or a terminal at a CPAM reception desk.
  • You can also call 3646.
