Where do I find my health insurance certificate of entitlement?

The certificate of entitlement can be downloaded from your ameli account, under “Mes démarches” then “Attestation de droits”.

Where to find it online

  • ameli.fr → Mon compte → “Mes démarches” → “Obtenir une attestation de droits” → download the PDF.
  • It serves as proof for an employer, a top-up insurer, a school or a prefecture.

If you have lost it

  • The document can be downloaded as often as you like from the ameli account or from a terminal at the CPAM reception desk.
  • With no account, call 3646 or go to a CPAM reception desk with an ID.
