Where do I find the form number of my vehicle registration certificate?

The form number is printed at the bottom left of the front of the carte grise, below the barcode, and starts with the year the document was issued.

Where to find it online

  • ANTS account → “Mon espace” → “Mes véhicules”: the carte grise is listed there once linked.
  • It is asked for when selling a vehicle (transfer declaration), changing address or getting a transfer code; it is different from the registration number (number plate).

If you have lost it

  • Without the carte grise, report the loss or theft on ants.gouv.fr and ask for a duplicate; the form number of the new document will be different.
  • The registration number, on the other hand, is on the plates and on the insurance: it is enough for most insurance procedures.
